Disease Management Coordinator Job Opening in santa barbara, California - Disease Management Coordinator Job Title/Position: Disease Management Coordinator Reports To: Clinical Supervisor JOB DESCRIPTION SUMMARY In collaboration with the Santa Barbara Diabetes Initiative, this position plans, organizes and implements home visits and diabetes self management education to a specific population of clients. Under an internal referral process, this position also provides home visit consultation to VNHC case managers for patients with the diagnosis of diabetes. Under direction, this position develops disease management processes, based on coordinated health care interventions and communications for populations with specific chronic care conditions, in which patient self-care efforts are significant in improving or maintaining health. This position will introduce the Health Coaching concept an assist in implementation of this model within the VNHC Home Health Case Management. The Disease Management Coordinator promotes the mission, values and vision of Visiting Nurse & Hospice Care. POSITION 1. Graduate of an accredited school of nursing with one to two years of recent acute care experience. 2. Current licensure as a registered nurse in State, CPR certification and valid driver?s license. 3. Bachelor?s degree with one year of home health care experience preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
