Care Transition Coordinator Job Opening in Abilene, Kansas - The Inpatient/OB Unit is seeking to hire a Care Transition Coordinator to work Monday through Friday, 7AM to 3:30 PM with some flexibility.
Position Summary:
Work with an interdisciplinary team to clinically assess and align appropriate levels of care, services, and programs during and after the patient's stay at Memorial Health System. Facilitate communication among patients, families, and healthcare staff. Provide follow-up care after hospital discharge. Build and support positive, effective relationships across the continuum of care and with the patients and communities served by MHS. Act as a resource to unit staff to coordinate quality care. This will be an evolving position, so applicant must be flexible and maintain willingness to continuous learning and skill development, be proactive about developing new areas of expertise.
Qualifications:
RN with active Kansas license
Bachelor's Degree preferred, Associate's Degree minimum
2 years hospital experience as an RN
Care management experience preferred, will train the right applicant
