Home Care PCW Job Opening in colorado springs, Colorado - Home Care PCW Rocky Mountain PACE (Program of All-inclusive Care for the Elderly) is seeking a PRN Personal Care Worker to work with our PACE participants in their homes. This position will assist participants with activities of daily living, restorative and supportive care necessary to the preservation of the home environment while assisting in monitoring the health status and providing a safe environment for participants. Candidate must have a high school diploma or equivalent. Must also have training as a PCW. A minimum of one year of experience with a frail elderly population required (experience with developmentally disabled adults will also be considered). Responsibilities include, but are not limited to: ◦assisting participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting); ◦assisting participants with restorative and supportive care (including ambulating, positioning, transferring, skin care, catheters, enemas, home exercise program as instructed by physical therapist) ◦assisting in monitoring participant's health status (vital signs, weight, skin condition, appetite and changes in functional ability) ◦reporting pertinent information to Home Care Coordinator in a timely manner ◦assisting participants with non-skilled ADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) ◦orders supplies from ADHC as needed ◦documenting services provided to participants consistently, accurately and in a timely manner via Electronic Medical Record ◦following procedure for flow sheet completion ◦providing a safe environment for participants and self-displays awareness of safety hazards and reports and/or reverses hazards ◦participating in participant's care planning through cooperation with multidisciplinary teams. The physical demands of this job include, but are not limited to: the ability to lift/carry up to x pounds and push or pull objects exceeding 150 pounds; standing, walking, pushing, bending, kneeling and reaching at arm's length for up to 95% of shift; working under stressful conditions. Must have excellent written and verbal communication skills, be proficient with Microsoft products, and possess strong computer and documentation skills. Candidate must be willing to submit to check, drug screen and check of driving history.
