LPN Intake Coordinator Job Opening in Columbia, South carolina - Compassionate Care Hospice (CCH) is actively seeking a full time LPN - Intake Coordinator for our Columbia, SC office.
POSITION SUMMARY:
The LPN - Intake Coordinator is responsible for the intake and coordination of appropriately referred patients. This position assists in gathering information to support the admission of the patient and implements the philosophy and objective of Compassionate Care Hospice.
QUALIFICATIONS:
LPN licensure, current state SC licenses required, minimum of three (3) years home health nursing experience, hospice experience preferred. Graduate of an accredited school of professional nursing or practical nursing.
Two years of general nursing experience to include current experience in the following: hospice, home care, med/surgery or critical care, IV skills and/or certification preferred. Recent hospice experience is preferred.
Excellent nursing judgment, assessment skills and documentation skills.
Excellent oral and written communications and interpersonal skills.
Demonstrated clinical competency in applying principles, methods and equipment used in nursing and appropriate specialty. Identified interest to work in specialty area.
Current CPR certification preferred.
Demonstrates sensitivity to the needs of the patients and caregivers (families, friends and significant others).
Ability to function as an effective member of an integrated healthcare team.
RESPONSIBILITIES AND DUTIES:
The LPN - Intake Nurse role establishes an independent, professional style of managing several functions.
Understands and is able to explain the hospice philosophy, practice of care and reimbursement to patient, families, discharge planners, physicians and other potential referrers.
Assists in the process of referral, getting all appropriate verification and signatures.
Includes all associated paperwork including nursing home and patient contacts within twenty-four hours of referral.
Establishes strong communication links and transfer process with the IDT members who will be caring for the newly admitted patient/families.
Provides complete and accurate documentation.
Assists the marketing staff in providing training and in-service education to nursing homes and other contracted resources.
Maintains up-to-date knowledge of hospice care, pain and symptom control and community resources.
Actively participates as a member of IDT, assists with implementation of an initial plan of care with Attending Physician, Social Worker and Medical Director.
Responsible for data entry of clinical information as required.
Participates in maintaining the integrity of the clinical record.
Participates in agency Quality Assessment and Performance Improvement tasks as assigned by Clinical Coordinator.
Performs other related tasks as requested.
