Registered Nurse Job Opening in Albuquerque, New mexico - Caring & compassionate RN needed to provide case management for patients receiving Home Health Care in their homes. Ensures that care delivery is within quality and utilization guidelines and is directed toward the achievement of desired clinical outcomes for the disease state. Responsible for development, coordination and scheduling delivery of care through a multidisciplinary team to the patient in the home or community setting.
Experience and Requirements:
Home Health Care experience
Current RN license
Flexible schedule, including availability to work evenings, weekends and holidays as needed
Dementia experience
Valid State Drivers License
Valid Automobile liability insurance
Reliable transportation
