Position Overview:
The Physical therapist organizes and conducts medically prescribed physical therapy programs to restore function and prevent disability following disease, injury or loss of a body part. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.
Essential Position Functions:
Applies diagnostic and prognostic muscle, nerve, joint, and functional ability tests to patients referred by a physician for physical therapy.
Plans and executes an individualized physical therapy program for each patient involving physical means, such as, exercise, manual therapy, gait training, muscle re-education, functional training. Uses a variety of activities and modalities to reach the stated goals for the patient.
Consults with other members of the rehabilitation team to create an individualized plan of care consistent with the needs, capabilities and discharge environment of the patient.
Educates patients/families regarding the patient?s condition, plan of care, home program, assistive devices and adaptive equipment as necessary.
Shares knowledge of physical therapy and promotes the profession to PT?s, PTA?s, and students. Participates in in-services and training for other medical and nursing personnel as indicated.
Ensures completion of all appropriate patient record forms and documentation. Documents objective findings to support the need for skilled services. Evaluates and records the patient?s progress for review by other members of the rehabilitative team.
Appropriately delegates patients to the PT Assistants and follows-up on those patients.
Minimum requirements include:
Graduate of an approved school of Physical Therapy (minimal requirement is a BS degree and eligible for licensure in New Hampshire).
