Full Time position (37.5 hours/week)
The incumbent in this position will have access to Agency Vehicles and may be eligible for tele-work.
Essential Duties:
Collaborate with physicians and inpatient clinical staff to identify appropriate individuals for care transitions services. Utilize established criteria, daily hospital census reports, and electronic medical records to facilitate in the daily referral process.
Conduct daily hospital visits providing information and guidance to the admitted individual and/or family for the acceptance to the care transitions program.
Report all individual declines to appropriate clinical staff, case management, or care coordinator for follow-up; as time permits, revisit individuals to encourage program participation.
Conduct home visits, Skilled Nursing Facility (SNF) visits, and follow-up phone calls within the guidelines of the Care Transitions program.
Minimum Qualifications:
At least 5 years of experience in Healthcare.
Familiar with a wide variety of community resources.
Bachelor?s degree in the Health Care Field or Care Coordination preferred.
Knowledge and Skills:
Knowledge of health care industry, caregiving, and chronic disease management.
Knowledge and appreciation of cultural diversity and low literacy issues in care provision.
Excellent verbal and written communication skills.
Knowledge of Microsoft Office including Word and Excel.
Special Requirements:
Valid Virginia driver?s license with acceptable record.
Attend trainings and maintain certifications as required.
Some evening work may be required.
Annual TB screening.
Annual influenza vaccine.
Salary is $17.54 hourly; $34,203 annually.
