Social Service Director Job Opening in Nerinx, Kentucky - The Loretto Motherhouse Infirmary is a licensed long-term care facility, owned and operated by the Sisters of Loretto, providing the highest standard of holistic care, security and dignity for all residents in a home like atmosphere. The Infirmary is located near Loretto, Ky and is the home of 85 residents.
The Social Services Director serves as a resident advocate, providing support, with a focus on holistic care, to residents, their families/healthcare surrogate and employees as appropriate. Participates in resident care planning to assist in identifying resident needs and preferences to promote resident quality of life and person centered care.
QUALIFICATIONS :
Associates degree or higher in Social Services related field or social services experience in long-term care.
Training or experience in holistic care of the geriatric population, with special emphasis on mental health, palliative care and dementia care.
Ability to exercise good judgment in balancing the needs of residents with their wishes and quality of life.
Must be tactful, courteous, patient, and cheerful when dealing with residents and staff.
Must possess leadership, planning, organizational and interpersonal skills.
Ability to work as a team player and to coordinate care with all disciplines.
Willingness to perform all duties in a manner that is consistent with the philosophy of the Sisters of Loretto and the mission of this facility.
Ability to protect privacy and maintain confidentiality.
Good writing and communication skills
Ability to operate a computer and use software such as: word processor, spreadsheets, clinical programs, and timekeeping programs.
Has keen sense of personal responsibility and accountability
ESSENTIAL JOB DUTIES :
Participate in pre-admission interviews and assessment. Recommend candidates for admission.
Coordinates admission process, ensures required forms are completed timely, collaborates with all departments to assist resident and family with the adjustment to long-term care. Provide care givers with life story information to make a smooth transition. Orients new residents and families to the facility, acquainting them with daily routine and services offered.
Recommend resident room changes; ensure room change notifications are completed, assist with resident room moves.
Compile and record information for MDS and Care Planning pertaining to cognition, mood, behavior, resident preferences, advance directives, and discharge planning.
Encourage a person centered care environment based on holistic care by demonstrating good judgment in the development of individualized care plans and supportive approaches in collaboration with all disciplines.
Coordinates resident discharge and provides community resource contacts as needed.
Document and investigate resident or family complaints/concerns and lost items, follow up as appropriate and track in a log.
Chair the Corporate Compliance and Ethics committees
Update Elopement/Resident watch list and communicate with staff
Oversee HIPAA policy and procedures
Oversee Resident Council Meetings
Assist residents with voter registration and absentee ballots
