Quality Assurance / Quality Improvement Licensed Vocational Nurse (LVN)
Cuidado Casero Home Health currently has an opening for a Quality Assurance / Licensed Vocational Nurse (LVN) to join our Home Health team.

Position Summary: The Quality Assurance LVN is a Licensed Vocational Nurse responsible for maintaining the quality of client care in accordance to the agency's objectives and policies, through effective planning, coordination, implementation and evaluation of the home health services provided. The position is responsible for assuring that standards of care delivered are being maintained by monitoring records, educating staff, and by using the continuous quality improvement process. Advised the Director of Nurses/Services of problems, concerns and recommended action.
QUALIFICATIONS:
Must be a Licensed Vocational Nurse in good standing and currently licensed by the state.
Must be CPR Certified.
Two to five years Home Health experience
2 years of Quality Assurance in Home Health
Experience in developing, implementing and evaluating continuous quality improvement programs and activities
Ability to demonstrate self-confidence and positive attitude toward self and others; maintains commitment and enthusiasm to goal achievement
Strong interpersonal and communication skills
Strong knowledge of Medicare Conditions of Participation
CHAP accreditation standards preferred
Ability to multi-task and coordinate simultaneous projects
Valid driverâ??s license
Good Computer Skills
RESPONSIBILITIES:
Review initial and ongoing documentation including plan of care, medical updates, therapy orders, physician orders and social work progress notes submitted by all home health staff members to evaluate regulatory compliance and quality assurance.
Identify staff members with documentation deficiencies and request corrections. Identify problem areas and instruct healthcare service providers in the area of documentation skills and regulatory updates.
Assist with the implementation of staff development processes.
Serve as a resource to DON and Field Clinical Staff.
Assist in orientation of new professional providers to documentation guidelines and requirements, when needed.
If needed, may provide direct nursing care as detailed in LPN Job Description.
Review documentation for compliance with frequency and services ordered.
Monitor and evaluate all aspects of care delivery to assure that Continuous Quality Improvement action plans are implemented and monitored.
Assist the DON with departmental staff meetings, case conferences and in-services as scheduled.
Actively participates on Quality Improvement Program Committee. Function as agency ADON in the absence of the DON.
