Referral assessments
Referral development for TLC
Train field reps in assessment and follow-up procedures
Qualify referral facilities and sources
Market the facility including TLC and Tideway
MAJOR DUTIES AND RESPONSIBILITIES:
Referral Assessment:
Receive referrals from admissions and referral sources. Gather records, meet with patient and family if present. Meet with medical staff, case manager, social worker, doctor, etc. to gather client info and establish referral rapport.
Conduct formal client assessment using TLC Cognistat and screening form. Grade Cognistat and forward to admissions in a timely manner.
Follow up with client, referring facility and admissions to enhance conversion likelihood.
Teach/train all new field reps the process of thorough on-site assessment of candidates, including chart review, clinical physical assessment, and interaction with TLC's admissions department, clinical director, medical director and department managers.
Referral Development for TLC:
Establish and maintain relationships with, as well as gain the confidence of, various referral sources throughout TLC's target market.
Act as liaison between referral sources and TLC.
Manage a territory and referral base through being a self-starter and organizing time and activities.
Respond to all referral calls in a timely manner.
Maintain a tracking and follow-up system for all referral calls and potential referral sources.
Maintain a goal of 20 face-to-face (one-to-one) calls per week. These are calls to physicians, social workers, discharge planners, utilization review managers, insurance case managers, etc.. Conduct all appropriate follow-up action.
Maintain up-to-date and accurate records on referral sources, including required information in Batchbook.
Develop opportunities for professionals, referral sources and family members to tour TLC's facilities.
Participate in, as well as promote to referral sources, all TLC sponsored conferences, seminars and workshops.
Qualify TLC Referral Facilities and Sources:
Call on facilities in market area to determine viability as referral source.
Complete Facility Contact Report and submit for data base.
Use information to focus resources for maximum referral development impact.
Marketing
Attend conferences and exhibits as determined by the Director.
Research and qualify facilities in catchment area for referral development.
Attend functions to network with potential referral sources and educate about TLC.
Provide CEU presentations to community and facilities in support of TLC mission.
General Departmental Duties and Responsibilities:
Complete weekly schedule on Google calendar for access and review by department staff.
Complete expense report for the week and submit to Director.
Attend local or state professional meetings that can expand your contact base and bring potential referrals.
During the day, when in the field or off site, be available by telephone or beeper. Communicate with Director of other department staff activities which cannot be left unattended.
Keep accurate records of sales calls and receipts for tax and reimbursement purposes.
Provide these items to the Business Development office on a weekly basis.
Design and develop instructional materials.
Develop, implement and deliver training needs.
Evaluate and analyze effectiveness of training by monitoring effectiveness of programs and progress of trainees and supervise the progress and performance of those involved in training.
Work closely with business partners i.e. admissions, clinical director, medical director or other department managers.
Support business operations.
Provide feedback to IT regarding challenge areas to improve productively and procedures.
Provide a professional, creative, energetic, and thorough training experience for coworkers.
Present technical information effectively one on one in a manner that is easy to understand.
Act as the Lead Nurse Planner and oversee adherence to TNA and ANCC COA criteria for all CNE activities in addition to preparation and orientation and training of all planners
Other duties as assigned by the Director.
Required Knowledge, Skills, and Abilities
General knowledge of the philosophy and implementation of marketing rehabilitation programs or health care programs.
Ability to prioritize and organize with minimum of supervision.
A knowledge of Microsoft windows, Excel, MS Word and a basic understanding of Batchbook.
Outstanding verbal and written communication skills.
Ability to track and report activities.
Understanding of general office procedures and telephone etiquette.
CPR certification
Education:
Minimum of Registered Nurse from accredited college or university.
Experience:
Minimum of 2-3 years experience in healthcare business. Prefer experience in dealing with major medical and/or catastrophic insurance.
