Under the direction of the Rehabilitation Manager an Occupational Therapist with HCS provides and directs the provision of home occupational therapy, based on agency policy and procedures, through the competent application of the therapy process. This is a Per Diem position. Serving Peterborough, NH and surrounding areas.
Responsibilities include, but are not limited to:
Performs comprehensive and subjective admission and ongoing assessment of client status that includes physical, psychosocial and environmental parameters.
Formulates individualized plan of care that incorporates analysis of assessment date and current scientific findings.
Provides skilled interventions aimed at achieving realistic outcomes within a specified time-period.
Modifies and updates plan of care to reflect progress toward goals.
Implements teaching appropriate to the client's needs and within the context of his/her environment and value system.
Plans, teaches and counsels regarding physical care, measures to promote , management, expected disease progression, social/emotional care and community resources.
Promotes client/family autonomy.
Qualifications:
Graduate of a program in Occupational therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association or its equivalent;
One year of professional Occupational Therapy experience under the appropriate supervision; or;
A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position.
Possession of a current State of New Hampshire license to practice as a registered Occupational Therapist.
Valid driver?s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
