Right at Home of Southern NH, a trusted provider of comprehensive, non-medical home care since 2008, is seeking a full-time Home Care Services Liaison based out of our newly opened Portsmouth, NH office to implement, oversee, and ensure delivery of the highest quality home care services tailored to the needs of clients, their families, and referral sources. We are seeking a self-directed, motivated, goal-focused individual who knows how to build relationships with referral sources and to help to increase and manage our client position will work closely with the Right at Home staff to ensure all clients are properly staffed and their care plans are completed and discussed with carefully matched caregivers. This position will be the primary liaison between our clients and our caregiver team.
The ideal applicant should be a proven healthcare professional with a minimum of two years experience in the home care field, preferably with a geriatric population, including supervising home care personnel and /or providing direct home care services.
Focused on day-to -day case management and the competency of the field care staff, as well as creative problem solving and conflict resolution, you will set the tone for your team's success. The Home Care Services Liaison must also possess excellent skills and be able to promote and foster ongoing collaborative communication with all internal and field staff, referral and community sources, healthcare professionals, as well as clients and family members.
In addition to traveling to client homes to perform client clinical assessments, prepare detailed, comprehensive care plans, and conduct follow-up and quality assurance visits, you'll be responsible for recruiting, retention, and oversight of the field caregiving staff (Licensed Nursing Assistants (LNAs) and trained Personal Care Assistants (PCAs) as well as assisting with necessary education, hands-on training, coaching, and mentoring in conjunction with the RN Operations Manager.
Job Skills/ Primary Responsibilities including but not limited to:
In this key position, you will be responsible for the overall delivery of services including leading the caregiver field staff in delivering exceptional client service and quality home care while ensuring adherence to the agency's policies and procedures.
Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
Maintain an ongoing caseload of clients
Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
Work with other team members to coordinate various aspects of a client?s care.
Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
Work closely with our exceptional caregiver team and office staff to ensure appropriate match and staffing of the qualified caregivers for our clients
Secondary Responsibilities:
Establishes and grows community relationships with referral sources (eg. hospital discharge planners; home health/hospice agency staff, rehab facilities, doctors' offices, assisted living facilities, etc.
Represents Right at Home at community events and marketing events as directed by Owner.
Assist with answering duties as needed as directed by RN Operations Manager
Assist with recruiting and retention of caregiving field staff
Participate in staff training and education.
Assist with scheduling caregivers
Requirements :
Nurse, Case/Care Manager, and/or college degree in human services, social work, or related field.
Minimum two years experience in home care, health care or senior-related industry; an equivalent combination of education and work experience may be considered.
Experience conducting best in class client/customer service
Must possess a valid driver?s license and personal vehicle for travel to clients, referral sources, networking events, etc.
Must be able to pass all background checks.
Available for emergencies or situations after hours/weekends depending on the needs of the business and clients.

Ability to utilize interpersonal skills in relating sensitively to the concerns affecting the elderly individual and the geriatric population as a whole.
Must demonstrate a passion for caring for seniors and prioritize their well being

Must demonstrate effective interpersonal skills including excellent oral and written communication skills as well as sound judgment and good decision-making skills
Must have the ability to work independently, maintain confidentiality of information and meet deadlines
Must have the ability to establish good working relationships with the office team, caregivers, and the community
Must present a professional appearance and demeanor and perform duties in a professional office setting
Must have proficiency in computer skills (Microsoft Office Suite). Will need to learn and master additional software including scheduling and telephony system.
Must demonstrate excellent organizational skills
