Health Home Care Coordinator Snohomish County Job Opening in Everett, Washington - The primary responsibility of the Health Homes Coordinator is to work with enrolled clients in Snohomish County, to develop and implement Health Action Plans that move clients along a pathway to improved quality of life, health and engagement. The coordinator will work with clients over time to seek and use resources that match the clients? needs as stated in the health action plan. This position is based in the field. The coordinator will work closely with the Health Homes Manager/Director and Health Homes administrative staff.
This is a remote position with daily travel throughout Snohomish County required and with up to 1 day per week required in the Seattle office - located at 1111 Harvard Avenue, Seattle, WA 98122.
Education:
Bachelor?s Degree in Social Work, nursing or related field preferred, or 4 years of experience in lieu of degree.

Current WA state driver?s license required.
Computer Skills:
Employee possesses intermediate operational knowledge of computers and comfort with web-based applications such as client management databases and payroll systems. Intermediate level of Word, Excel and Outlook required.
Travel:
Must have reliable transportation, a clean driving record, be able to drive and motor vehicle insurance that meets state required minimums.
Physical Requirements and Working Environment:
This position requires the manual dexterity sufficient to operate phones, computers, and other office equipment.
This person must speak clearly using the English language to accurately convey information and be able to hear at normal speaking levels both in person and over the telephone. Vision correctable to 20/40 is required to review written materials. The working environment may vary as needed to accommodate changes and growth of the department.
Qualifications and Requirements:
At least 1 year experience in social services or care coordination and/or collaborative initiatives, preferably related to healthcare access.
Experience working with people with mental illness and/or substance abuse preferred.
Ability to work independently and in cooperation with others.
An energetic and positive approach to the rapidly evolving changes and challenges of a complex client population.
Comfort and competence working with a diverse client base; age, gender, race, language, country of origin, etc.
Essential Duties and Responsibilities:
Outreach and engage clients.
Work with clients, families and others to develop Health Action Plan (HAP) within deadlines.
Promote progress on steps in Health Action Plan (HAP).
Complete required documentation within deadlines.
Increase clients? ability for self-management and shared decision making.
Connect clients to relevant community and health care resources, with the goal of enhancing patient health and well-being and increasing client satisfaction.
Improve continuity of care by helping patients navigate relationships with providers and outside resources.
Attend on-site internal meetings as required, including 1:1s, department meetings, staff meetings, etc.
Maintain appropriate confidentiality of all patient health information.
Travel to client location in varied environments required.
