Case Manager Job Opening in Louisville, Kentucky - Seeking a Case Manager to service Jefferson, Scott, Owen and Franklin counties.
Case Managers work to assist clients and families with alleviating problem behaviors by:
-Linking your clients with services that are tied directly to the client's diagnosis.
-Providing assistance in coordination of treatment through monitoring services.
-Linking client and family to needed community resources and referrals.
-Helping prepare client and family to function independently after services are complete.
Education Requirements:
Bachelors degree in Human Services related field plus one year of post graduate experience
