RN Case Manager Job Opening in Shawnee Mission, Kansas - Compassionate Care Hospice is actively seeking a full time Registered Nurse (RN) Case Manager for Shawnee Mission, KS program.
Summary:
The RN Case Manager works as an intricate part of the clinical team where the primary responsibility is to coordinate the overall plan of care for each patient in conjunction with the attending physician.
Qualifications:
The RN Case Manager will possess excellent communication skills (written and verbal) as well as strong interpersonal and organizational skills. Hospice experience is preferred . He/she will be a self-motivated individual who works well in a team environment and is able to work flexible hours (and ?off" hours) as may be needed.
Requirements:
Must be a Registered Nurse and require KS & MO license
Graduate of NLN accredited school of nursing
