About this job: As a member of the COR Health Pediatrics Team, the Nurse Coordinator provides support to all the Company's departments and assists in supporting clients, family, staff and organizational goals. Provides general oversight of data entry under the direction of the Director of Nursing and / or Administrator. Responsible for ensuring COR Health is compliant with all aspects and levels of Federal, State and local laws, regulations and standards.
DUTIES AND RESPONSIBILITIES (other duties may be assigned) :
Communicates directly with doctors, families, clients and referral sources to ensure that care is appropriate and of quality.
Perform all documentation requirements to meet the Company's policy and regulatory requirements.
Manage client documentation including updating plans, updating medical profiles and writing doctor?s orders.
Responsible for a client case load that requires performance of client care, including, but not limited to, opening cases, visiting clients, supervisory visits, and Medkid assessments.
Assist in auditing all home health client records to meet the Company's Quality Assurance / Quality Improvement requirements as assigned.
Perform monthly calls to all staff who work within the department to obtain updates, concerns, and conduct surveys.
Assist the Director of Nursing and/or Administrator with management of client file preparation, client monthly paperwork mailings and documents tracking as assigned.
Input client and staff information into the Company's client database as required.
Coordinate with all nursing staff and clients to ensure that their scheduling needs are being met.
Develop and maintain strong relationships with referral sources, other agencies and regulatory bodies within the community.
Maintain confidentiality in accordance with the Company's policy and regulatory requirements.
Work select RN block shifts until build a full caseload is assumed.
Enjoys working in a fast paced environment and being part of a collaborative team.
SCHEDULE: Office hours are Monday through Friday, 8:30am - 5:00pm.
LOCATION: Augusta / Lewiston Area. For the initial 90 days, a candidate would be spending time at the Corporate Office located in Portland, Maine.
EDUCATION, LICENSE, AND/OR CERTIFICATIONS REQUIRED: Registered Nurse and possess an active Maine State Board of Nursing License. Graduate of an accredited school of professional nursing. Minimum of 1 year of RN experience. Supervisory or management experience of at least 1 year highly desired. Experience with data entry and have strong computer knowledge, including Microsoft Office (ie. Excel / Word) , and have exceptional grammar/spelling abilities.
