Community Liaison Job Opening in Flagstaff, Arizona - Welbrook Senior Living LLC is seeking experienced, dynamic, candidates for the position of Community Liaison of the newly constructed 50 bed private room Rehabilitation and Skilled Nursing Facility, Welbrook Senior Living Las Cruces, LLC in Las Cruces, NM .
The Community Liaison serves in a consultant role, acting as an advocate and marketer of our services to hospital discharge planners, and physicians. The purpose of this position is to ensure referral goals are met and the facilities receive proper referral of residents.
JOB SPECIFIC RESPONSIBILITIES:
Demonstrate knowledge of admission department standards.
Assess each prospective resident to determine the appropriateness for admission.
Acts as Liaison for hospitals and fosters relationships to ensure referrals.
Manages primary referral hospitals and ensures a smooth discharge process for patients transitioning from a hospital setting to our centers.
Meet referral/admission goals as set by Administrator.
Develops strong relationships with key referral hospital personnel and identify new opportunities for partnering and growth.
Reviews medical documentation, working with physicians to make clinical judgments, and coordinating care with the nursing and admissions department.
Case manager for HMO, private insurance and other types of payor sources that require.
Inform facilities of any special requirements for
Promotes effective relationships and communication with all staff, residents and families.
Promotes going outside of the facility to make contact with any healthcare providers who may benefit from your facility?s services. These contacts include nurses, physicians, case managers and social workers.
Comply with the number of admission required as monthly goal by Supervisor.
Performs any miscellaneous work assignments as may be required.
Makes independent decisions pertaining to the admission department without supervision from immediate supervisor.
Uses discretion and independent judgment as it relates to admission department in a timely and efficient manner.
