Case Manager Job Opening in Las Vegas, Nevada - The primary role of a case manager is to provide patient and family education about their disease process, medication, and treatment received up to including discharge planning.
Job Duties/ Responsibilities
Meet all new admissions.
Conduct a comprehensive prior level of function assessment for each new admission via interviews and chart reviews.
Coordinate healthcare service delivery and necessary medical follow-up.
Assist the team with planning a safe and effective discharge.
Evaluate an individual safety and ability to live independently at home or other setting.
Educate patients and families to ensure safe discharge which includes education on functional maintenance programs, disease management, and physician?s follow-up.
Perform a routine check on insurance and payor source daily.
Assess and review change of condition changes and care concerns to ensure good customer service.
Attend change of condition meeting and care planning daily and as scheduled.
As a part of PPS meeting, review ARD selection, ADL flow sheet accuracy, rehab services and estimated RUG with assessment indicator.
Schedule and conduct case management meeting weekly.
Review and manage MDS assessment detail report.
Completes manage care updates and concurrent review report.
Contact families as needed to discuss patient status along with any changes in the care plan
Assist the business office in validating the accuracy of insurance claim.
Make contact with primary referral sources to review patient outcomes with discharge planners, case managers and other health care professionals.
Other duties assigned by administration to improve and deliver quality of care to the residents.
