Quality Improvement Assistant Job Opening in Hope Valley, Rhode island - Essential Functions Statement(s)
Responsible for tracking quality measures for the organization
Responsible for reviewing and entering data into the electronic health record system (EHR).
Develop and/or coordinate information necessary to perform crucial audits using the process driven format with appropriate benchmarks and measurable outcomes.
Follow a set predetermined schedule for organizational audits.
Maintain proficient documentation of QI activities.
Conduct center- specific discussions with appropriate staff regarding any identified deficiencies or areas of desired performance or site improvement initiatives.
Maintain all necessary reports for all clinical quality reporting, UDS, Meaningful Use, CTC, RICCC, CMS, Medicaid, ACO and any and all other payers or initiatives.
Participate in design and implementation Quality Improvement measures that will augment and support the overall systems of Wood River Health Services.
Responsible for obtaining results and records as it pertains to clinical utilization
measures, including phone calls to patients, checking schedule to ensure appointment was kept and reviewing EHR notes.
Assists in reviewing gaps in care measures work to improve the quality outcomes.
Act as a patient navigator to identify and provide individualized assistance to establish clients reduce barriers to accessing and completing Breast, Cervical and/or Colorectal Cancer Screening (CRC).
Participates in the Quality Management Committee meetings.
Assist the Care Management and Community Resources teams as needed
Participate in professional development by attending programs workshops, in-services, assigned committees, department and staff meetings.
Maintain strict confidentiality in all matters.
Adhere to organizational policies and procedures.
Enliven and support the mission, vision and values of WRHS.
Performing other duties as assigned within scope of practice.
QUALIFICATIONS & EXPERIENCE:
in healthcare related field preferred.
At least 2 years of experience that is directly related to the duties and responsibilities specified.
Experience with Microsoft Word, Excel, and PowerPoint.
Experience with electronic medical records.
Excellent communication skills.
Able to work under tight deadlines.
Able to exercise imitative and display good knowledge of regulatory requirements, UDS, Meaningful Use, NCQA, CTC and RICCC.
Able to collect and analyze QI data in a timely manner and summarize for presentation purposes.
Certified Health Source RI Navigator preferred or must obtain certification at first opportunity after starting.
