The ideal candidate for the Aging Life Care Specialist is passionate about supporting the goals and objectives of the Senior Independence Service Area through serving as an advocate and liaison for seniors and their families. This position will develop and monitor individualized care plans that are personalized and ongoing to meet seniors? challenging needs, improve quality of life, and promote independent living.
The Aging Life Care Specialist will identify and bridge the gaps between the healthcare setting and a senior?s home, independent living, and long-term care. This position will assess client strengths, needs, and preferences in identifying resources and coordinating and maintaining continuous care plans. The Aging Life Care Specialist will network with service providers and resources to allow for cohesive and uninterrupted service delivery to clients.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Program & Fiscal Management: Responsible for day-to-day activities throughout the assigned area of the program; Facilitates and participates in required meetings; Works with internal and external stakeholders related to program issues; Participates in legislative hearings as needed or as requested; Serves as point of contact for referring agencies as needed; Monitors program budget and monthly reports; Coordinates and collaborates with partners and other resource and referral services; Engages community and builds program awareness through presentations and discussions related to senior care; and Collaborates with local and state associations to ensure best practices.
Service Delivery: Develops and conducts comprehensive senior assessments to identify strengths, needs, support systems, concerns, and preferences; Develops and implements initial care plans and establishes goals; Maintains, monitors and changes care plans based on provisions and needs; Provides crisis intervention and consultation as needed; Consults with treatment partners to ensure effective service delivery to clients and their families; Serves as advocate and liaison for seniors and families related to direct care services; Coordinates necessary support with referral sources; and Makes recommendations related to individual treatment as needed.
Recordkeeping: Completes necessary paperwork including intake, assessment, care plans, goals and other required documentation; Prepares documentation on accordance with Agency and State and Federal regulations; Completes paperwork as needed; and Completes reports as requested by Agency and/or external agencies for program monitoring. _
Community Engagement: Markets Aging Life Care Program to individuals, families, and groups; Develops and distributes program materials; Participates in senior events including booth set up and facilitation; and Presents and facilitates program discussions with senior groups, veterans, tribal councils, churches, and others as needed.
Other: Attends meetings as required; Assists Supervisor and/or other related programs with special projects and other assignments; and Other duties as assigned.

Bachelor?s Degree in Gerontology, Nursing, Social Work, Human Development & Family Science or other Human Services related field
2 years of case management experience with elderly population and experience working with community resources, or equivalent of
Ability to become certified in care management within 12 months of employment
Valid Driver?s License
MENTAL/PHYSICAL DEMANDS
Up to 50% travel * Varied/flexible work schedule including days, evenings, nights, and weekends
Standard movements with limited physical exertion required. Movements are consistent with what would be found in a normal office setting including exertion of light physical effort for short periods.
Usually normal concentration and deadlines with periodic periods of sustained deadline pressure. Work may involve changing problems, circumstances or demand.
Work involves minimal risk to personal safety of injury, illness or health problems arising from the environment or the public/clients.
