Abode Home Health is seeking a compassionate, dependable Registered Nurse for our Denver branch.
We are growing and need to hire a full time registered nurse (RN) case manager to care for our patients. We will pay a $5,000 sign on bonus.
Job Duties of this rewarding position as a Home Health Registered Nurse (RN) Case Manager include:
Performing in-home or place of living evaluation and establishing an individualized care plan and medical record baseline for each patient.
Collaborating with physicians and other agency staff to implement and coordinate the patient's plan of care.

On-going training and clinical support make our Home Health Registered Nurses able to work independently while providing care to a wide range of complex cases.

Requirements:
Valid Colorado Registered Nurse License
Valid Colorado Driver's License
Proof of Car Insurance
Great Clinical Assessment Skills
Strong desire to serve others and to enrich lives
Previous Home Health Registered Nurse - RN - Case Manager experience not required but preferred.
