Nurse Practitioner Job Opening in Wichita, Kansas - POSITION SUMMARY
Under the supervision of the Medical Director, and in coordination with the Clinic Manager for administrative functions, the Nurse Practitioner (NP) provides primary medical care to program participants, including performance of initial comprehensive assessments, periodic reassessment and unscheduled assessments; develops the plan of care in collaboration with the interdisciplinary team; provides direct participant medical care, and evaluates the effectiveness of the plan of care. The NP will function under a collaborative practice agreement with at least one primary care physicians. The NP will, in conjunction with the other health care professionals, make independent decisions regarding medical needs and health regimens. Carries out medical procedures as indicated within the NP scope of practice. This position has direct contact that provides care to all populations.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Assessment
Participates as a member of the interdisciplinary team (IDT) and in initial, semi- annual, unscheduled, and annual assessments; attend morning updates and report changes in participants? baseline status to appropriate staff on a daily
Provides initial and ongoing participant assessments, care plan development, and implementation of care
Obtains a complete health history and record the findings in a systematic organized manner.
Maintains current, accurate documentation of health care services provided, coordinated, or contracted
Performs physical examinations, order and evaluate appropriate laboratory and diagnostic tests, and record results in a systematic
Identifies medical, social, cultural, and financial
Performs functional status evaluation and/or mental status examinations using standardized
Identifies and describes behavior patterns of the chronically
Triages status changes in participants; and facilitates appropriate management of problem identified.
Treatment
Provides preventative health care and health promotion for PACE program participants.
Treats and manages acute chronic
Regulates and adjusts medications as
Facilitates medical specialist consultation as
Educates participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing
In conjunction with the primary care physician provides inpatient attending coverage responsibility when participant is hospitalized or admitted to a nursing facility and communicate participant status to the PACE program

EDUCATION AND/OR EXPERIENCE
Master's degree (M.N. or M.S.N.) as NP, or Nurse Practitioner with APRN in state which
Minimum one (1) year recent successful experience working with a frail or elderly population in a community-based or long term care

Position is Category I of Potential Exposure. Use of appropriate Personal Protective Equipment (PPE) is required for every employee engaging in Category I tasks.
