With general direction from the Clinical Supervisor, the Youth & Family Support Specialist (YFSS) works closely with the Masters Level Home-Based Clinician in providing clinical treatment, support, assistance, and case management to children, youths and families at risk and/or with a history of psychiatric or substance abuse disorder. The YFSS provides clinical services to clients and coordination support to the agency. Children, youth and families will be self-referred or referred for services by DCF, DMH, state agencies, schools or other providers.
Responsibilities
The Youth & Family Support Specialist will:
In conjunction with Senior Youth & Family Support Specialist, be responsible for assessments, treatment and safety planning
Provide services to the child, youth and family
Provide clinical case management and linkages to community supports, resources and opportunities
Provide intensive family treatment and support interventions that assist the youth and family to overcome challenges posed by behaviors associated with the youth and family?s condition
Coordinate care with schools, service providers and other entities that provide support to assure that the approaches taken are integrated and clinically coherent
Attend care plan meetings when appropriate; facilitate team meetings with collaterals and attend case reviews as needed
Ensure progress notes are completed within NLCWC?s mandated period
Meet productivity benchmarks and KPI?s as assigned by NLCWC?s productivity plan
Participate fully in all individual and group supervision meetings
Submit work schedule to Admin staff
Attend internal supervision and mandated trainings scheduled by NLCWC, remain current in the field to pursue professional growth and development
Meet service expectations and deliverables set by providers, state agencies and community supports
Communicate with DCF and external providers when delivering services to families referred by DCF for Family Support and Stabilization Services (FSS)
Establish collaborations with providers, state agencies, schools and other community and natural supports
Will require working nights and weekends
Other duties as needed
