Coastal Medical is searching for a full time Nurse Practitioner to be a part of our Coastal @ Home team providing comprehensive home-based medical care to our highest need patient population. Working in collaboration with a dedicated team of professionals, the Nurse Practitioner will provide physical assessment, management of health problems, health counseling and treatment for patients in their home. Our position offers a unique opportunity to our patients' overall health through our primary commitment to wellness and disease prevention. This Nurse Practitioner will be part of a multi-disciplinary team that cares for the Coastal Medical geriatric population.

Duties and Responsibilities

Responsibilities include but are not limited to:
Conducting in-home medical visits to geriatric and adult patients for routine visits, acute illness, post hospital or sub-acute discharge and routine follow up care for patients with complex medical conditions
Conducting goals of care discussions with patients, as necessary and facilitating the completion of advance directives documentation
Collecting and documenting past medical history and family history
Identifying gaps in care, ensuring optimal care coordination, and providing teaching and disease management to patients as needed
Establishing immunization schedules
Ordering and reviewing lab results
Establishing an accurate problem list for patients and supporting documentation of illness and risk factors
Coordinating care with Primary Care Healthcare team and participation in Clinical Care Conferences
Referring patient to community programs for weight loss, exercise, fall preventions and other services as appropriate

Job Requirements
Current Nurse Practitioner (APRN or NP) licensure in the state of Rhode Island (in good standing required)
Active Board Certification as an Adult, Geriatric or Family Nurse Practitioner
Outstanding organizational skills and ability to multi-task
Ability to utilize an electronic health record
Initiative, problem solving ability, adaptability and flexibility
Ability to work without direct supervision and practice autonomously
Capacity to collaborate with professional colleagues as necessary to ensure quality care
Presents an authentic, friendly, welcoming a nd professional public facing appearance
State-wide travel and operation of motor vehicle for extended periods
