Licensed Practical Nurse -Extended Care Job Opening in Alexandria, Louisiana - Licensed Practical Nurse for field: Seeking a Licensed Practical Nurse/LPN who will be responsible for providing direct patient care under the supervision of a registered nurse. Will be responsible for working collaboratively with the members of the healthcare team to help meet positive patient care outcomes.
The LPN will be responsible for direct patient care of one patient in the patient's home.
Responsibilities of Licensed Practical Nurse/LPN include but not limited to the following:
Implement plan of care initiated by the clinician
Provides accurate and timely documentation
Assess and provides patient and family caregiver education and information pertinent to diagnosis and plan of care
Participates in coordination of home health services to the clinician and/or clinical supervisor
