Regional Quality Assessment and Performance Improvement Coor... Job Opening in Parsippany, New jersey - Compassionate Care Hospice (CCH) is actively seeking a full time Regional (QAPI) Quality Assessment and Performance Improvement Coordinator based in Parsippany, NJ.
This position requires to travel to offices to provide QAPI support for Michigan, Illinois, Pittsburgh, Ohio, and Wisconsin. Quarterly travel to one of the five offices to rotate through the offices would be needed in order to conduct on site QAPI meetings, the other meetings are done remotely each quarter.
POSITION SUMMARY:
Coordinates and implements the organization?s QAPI Program for multiple locations.
QUALIFICATIONS:
Graduate of an accredited school of professional nursing. BSN preferred
Currently licensed as a RN/LPN through State Board of Nursing
Hospice Experienced Preferred
Minimum three years experience in Quality Assessment and Performance Improvement
Ability to communicate effectively with other members of the health care team
Excellent oral and written communications and interpersonal skills
Computer literate
Licensed driver with reliable automobile which is insured in accordance with state law and organization policy
Is flexible, cooperative and is self-directed in fulfilling role obligation at all levels within the organization
Familiar with applicable State and Federal regulations, licensure and accreditation standards
Strong presentation, training and teaching skills
Demonstrated ability to work with staff at all levels
Demonstrates good communication skills
RESPONSIBILITIES AND DUTIES:
Responsible for implementing the Quality Assessment and Performance Improvement Program of their assigned multiple locations.
Responsible for collecting and analyzing data of their assigned multiple locations.
Assures monitoring, and standardization of clinical records for completeness and accuracy, concurrently and retrospectively utilizing established outcome criteria, for example, plans of treatment and plans of care.
Responsible for evaluating and monitoring the quality of patient care and service rendered by all staff members and the hospice operations of their assigned multiple locations.
Participates in employee education and orientation related to the QAPI Program.
Functions as a resource to staff of all levels.
Assist Program Director in assessing, planning, implementing and evaluating the Quality Assessment and Performance Improvement Program of their assigned multiple locations.
Responsible for evaluating and monitoring the Performance Improvement Projects for their assigned multiple locations.
Arranges for Quality Assessment and Performance Improvement quarterly meeting for multiple locations and reports findings to the Professional Advisory Committee and Corporate Compliance and Quality quarterly.
Formulates the reports for the performance improvement committee and reports quarterly to the National Quality Assessment and Performance Improvement Officer.
Functions as their assigned organization?s resource on Medicare/Medicaid certification, licensure, CHAP and National Hospice organization and other applicable standards.
Maintains, audits and reviews the Quality Assessment and Performance Improvement records.
Establishes and monitors ongoing peer review program.
Responsible for providing assistance with audit teams, providing them with appropriate certification, accreditation and licensure information.
Assists and prepares, as needed, special reports related to Quality Assessment and Performance Improvement for the Professional Advisory Committee and the assigned organization annual report.
Assists in the development of critical pathways and outcome criteria.
Ongoing assessments of education needs based on Quality Assessment and Performance Improvement findings.
Responsible for the timely, accurate, reporting of data to health oversight organization.
Review and analyze health oversight organization quarterly report.
Performs other tasks as requested.
Prepares reports for CMS Reportable Quality Measures for assigned multiple locations.
Ability to schedule travel arrangements to assigned multiple locations.
