Complex Care Nurse Coordinator Job Opening in Somersworth, New hampshire - Complex Care Nurse Coordinator (Full-Time/40 Hours per week)
We are looking for an enthusiastic, compassionate and detail oriented Complex Care Nurse Coordinator to join our team for 40 hours/week.
The Nurse Case Coordinator will be responsible for coordinating the care of our most complex patients and assisting our patients in navigating their transitions through the healthcare system. The Nurse Case Coordinator will be expected to work collaboratively with all members of our interdisciplinary health care team, as well as with specialists and social service providers in the community. The ability to track, monitor and impact both clinical outcomes as well as healthcare utilization data for this patient population will be essential.
Minimum Knowledge and Skills:
Experience with health promotion, chronic care management, case management and patient education
Knowledge of, and interest in, working in a Patient Centered Medical Home (PCMH) Model of Care
Bachelors Preferred
Certification in Case Management Preferred
Current NH RN license
Proficient Computer skills, experience with using EMR
Skill in assessing and evaluating the existing resources to meet a patient?s needs
Ability to work independently as well as a proven track record of being a team player
