Case Manager Job Opening in Manchester, New hampshire - Case Manager, New Horizons Homeless Shelter
New Horizons operates a shelter for adults experiencing homelessness. It is the organization?s goal to work with our shelter guests to help them find the strength and skills to move on to self-sufficiency. The Case Managers assist guests to prepare an action plan establishing goals enabling the client to establish sustainable housing outside the shelter ending their homelessness.
The Case Management process begins by identifying the factors that caused the client to become homeless and directing that client toward steps that may address those factors including health assessments by the on-site Healthcare for the Homeless Team and the Greater Manchester Mental Health Outreach Team. The Case Manager helps determine the client?s eligibility for various housing options, government benefits, work training opportunities, life skills development programs, and may direct clients to seek employment. Once a client has prepared their goal plan the Case Manager encourages them to act on that plan.
Specific duties include:
Performs case management intakes;
Assesses client needs and develops an action plan to reach identified goals;
Monitors client progress in meeting goals and provides appropriate level of support;
Advocates and serves as liaison for clients working with other agencies, as well as within
NHNH, including medical services;
Maintains files on clients and generates reports as required by supervisor and state
Regulations;
Attend and actively participates in staff meetings and works cooperatively with others;
Supervises volunteers and student interns receiving practicum experiences at New Horizons;
Participates as an active member of local professional organizations or associations and serves on committees as requested and with the approval of the Program Director;
Adheres to established policies and procedures of New Horizons;
This is a full time salaried position on a schedule of four 10-hour days Monday ? Thursday paid based on experience.
Requirements:
Bachelor?s degree or other human services field with 3+ years experience. Must be proficient in Microsoft office programs and have the ability to work with a diverse clientele in the midst of a traumatic state of experience.
The chosen candidate will also be required to submit to a criminal background and motor vehicle record check and must have a valid driver?s license.

Salary: $30,000.00 to $37,000.00 /year
