Wound Care Nurse Job Opening in Lexington, Kentucky - RN or Certified Wound Care Nurse need for our beautiful campus. Mon-Friday with on call rotation.
FUNCTION: To coordinate and facilitate aspects of care related to wounds throughout the facility.

Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in the position. Certificates, Licenses, Registrations: Current state license to practice as a nurse. Current CPR certification and additional certification in nursing specialty desired. MATHEMATICAL SKILLS: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: *
REASONING ABILITY: Ability to define and solve problems, collects data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical journals and deal with several abstract and concrete variables.
LANGUAGE SKILLS: . Ability to read, analyze and interpret, professional journals, technical procedures or governmental regulations. Ability to communicate orally and through written reports and other documents relating to Resident, staff and others. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers and the public.
Care Functions:
Provides consultation and direct assistance in the management of residents with altered skin integrity (pressure necrosis, wounds, fistulas, etc.)
Collaborates with Rehab, nutrition services, physicians and nursing staff to develop a plan of care for wound patients/residents.
Provides wound care (including debridement) in accordance with state regulations.
Develops basic skin care programs for the facility, including risk assessments, prevention and treatment protocols, and mechanism for ongoing monitoring. These programs are to be updated as needed to ensure compliance with current wound management principles and Five Star Quality Care, Inc. standards.
Provides in-service programs as needed to assist nursing, rehabilitation, and medical staff to maintain knowledge and competence in wound care.
Collects, analyzes and documents information and statistics related to wounds in the facility.
Serves as consultant in the management of residents with wounds.
Performs clinical evaluations for potential wound admissions.
Participates in marketing presentations.
Reviews wound and skin care products in Central Supply on a continuing basis and recommends revisions as appropriate.
Participates in product research relative to wound practice.
Participates in Continuous Quality Improvement projects relative to wound and skin are issues.
Identifies problems with implementation of wound and skin programs and takes steps to rectify these.
Analyzes the financial component of wound care and makes treatment decisions and recommendations that are cost effective.
Maintains expertise in wound care nursing.
Care and Services :
Uses a systematic approach to provide individualized nursing care.
Performs nursing assessments regarding the health status of the resident.
Contributes to the resident?s assessment (MD/RAP?s) and the development of a plan of care.
Makes nursing diagnoses which serve as a basis for the strategy of care.
Develops a plan of care based on assessment, implementing nursing care.
Evaluate the resident?s responses to nursing intervention.
Selects and institutes appropriate nursing intervention that might be required to stabilize a resident?s condition and/or prevents complications.
Consults with physician or licensed independent practitioner.
Clarifies any order or treatment regimen believed to be inaccurate, non-effective or contraindicated by consulting with the appropriate licensed practitioner and notify the ordering practitioner when making the decision not to administer medication or treatment.
Knows the rationale for the effect of medications and treatments and correctly administers the same.
Accurately reports and documents the resident?s symptoms, responses and status.
Designs, promotes and organizes resident education and counseling based on health needs.
Collaborates with the resident, members of the healthcare team and, when appropriate, the resident?s significant other(s) in the interest of the resident?s healthcare.
Consults with, utilizes and initiates referrals to appropriate community agencies and healthcare resources to provide continuity of care.
Plans, organizes, manages and coordinates nursing nursing care:
Initiates nursing interventions which promote resident?s ability to adapt and adjust to living as independently as possible.
Includes skill practice in such activities as walking and mobility, dressing and grooming, eating, swallowing, bowel and/or bladder retraining, transferring, amputation care and communication.
Seeks to improve or maintain resident function in physical abilities and activities of daily living.
Plans, schedules and documents specific nursing nursing interventions in the care plan and in the resident?s clinical record.
Performs or supervises members of the nursing staff who carry out nursing nursing activities.
Incorporates training and skill practice activities which include repetition, physical or verbal cueing and task segmentation.
Coordinates and plans nursing interventions with therapies (PT, OT, ST and RT).
Keeps others informed about the status of resident?s receiving nursing rehabilitation/ restorative nursing care.
Reports change in resident condition and nursing nursing needs.
Teaches nursing assistants the use of nursing nursing and prevention measures.
Follows procedures related to the use and maintenance of nursing nursing equipment and supplies
Documents the nursing nursing care and services provided to each resident. * Makes resident rounds every two hours to assess physical and emotional status and to initiate any required nursing interventions.
Responds to resident and family concern; ensures that each concern is documented and a resolution is initiated.
Coordinates admission, discharge and transfer of residents.
Obtains report from nurse he or she is relieving, provides report to nurse coming on duty and keeps the director of nursing service or others as appropriate informed about the status of resident and related matters; performs and supervises complete, timely and accurate documentation of resident care.
Performs or supervises the review of medication and treatment records for completeness, accuracy in transcription of physician orders and adherence to stop order policy.
Performs or supervises the administration and documentation of medications, enteral nutrition and treatments per the physician?s order and accurately records all care provided.
Performs or supervises the ordering and receipt of medications.
Performs or supervises the serving and documentation of prescribe diets and fluid intake.
Performs or supervises the count of all narcotics, signing for count and exchange of keys to medication carts and medication room.
Checks emergency equipment (oxygen and suction) and emergency medication kit (if application).
Demonstrates procedures related to the use of nursing equipment and supplies.
Accepts only those nursing assignments that are commensurate with one?s own educational preparation, experience, knowledge and ability; obtains instruction and supervision as necessary when implementing nursing procedures or practices.
Performs other duties or functions as directed by the Director of Nursing.
