Case Manager Job Opening in New Iberia, Louisiana - Ageless Healthcare has a Case Manager position available! We are a home healthcare business! You must have experience being a case manager or doing similar work such as direct service work/CNA work. Case management experience preferred. Be somewhat familiar with the LT-PCS, EPSDT, CCW, NOW, and SIL program rules and must have great organizational habits, willing to be a networker, and work well with others. Be computer literate. Experience in Microsoft Word and Excel is necessary. Be available to be on call 24 hours a day, 7 days a week.
