We are seeking a full time (36 hours per week) Licensed Practical Nurse (LPN). The LPN will join our Resident Care department in promoting the health and well-being of each resident by providing; accurate evaluations, giving appropriate input to the development of service plans, educating staff on appropriate and proper care for each resident and overseeing the healthcare needs of all residents. The LPN will be responsible for implementing and following state and federal regulations and company policies and procedures. Must be able to work closely with the Administrator, RN and Resident Care Coordinator to develop and enhance the health capabilities of residents and have a care philosophy that is resident centered. Must be a teacher, problem solver, good listener and able to think and work independently. Weekend and on-call capabilities a MUST.
Job Description
Participates in the interdisciplinary team and works cooperatively with team members to ensure the coordination of care and treatment for residents
Develops and updates the Resident Assessment and Comprehensive Care Plan as resident conditions change
Attends treatment and care plan meetings as needed
Assists with charting of all clinical and nursing care provided to residents
Ensures charting is accurate, done in a timely manner, and meets established standards and regulations
Provides consultation, information, and support to residents and their families as needed
Ensures information is transferred regarding resident care and condition as related to nursing services at shift change report (i.e. shift beginning and shift ending)
Participates in in-service and continuing education programs, staff meetings, and resident conferences as requested by immediate supervisor
Provides emergency care as appropriate, in accordance with physician orders and advance directives
Oversees the management and implementation of resident care plans and the documentation in care plan of resident progress and changes in condition
Ensures that all safety and infection control practices are followed
Provides care in accordance with federal, state, and local regulations
Performs other duties as assigned
