Job Description Summary
Full Time Registered Nurse (RN)
Job Responsibilities
Perform initial and ongoing assessment of the impact of the terminal disease on the patient?s physical, functional, psychosocial, and environmental needs
Implement the individualized POC and recommend revisions to the plan as necessary
Consult with and educate the patient/family regarding disease process, self-care techniques, end of life care, nutrition and dietary needs
Prepare clinical and progress notes that demonstrate progress towards established goals
Inform physician and other personnel of changes in the patient?s needs and outcomes of intervention, while evaluating patient/family response to care
Determine scope and frequency of services needed based on activity and patient/family needs
Assess ability of the caregiver to meet the patient?s immediate needs upon admission and throughout care
Communicate information using current process and technology available to the organization
Qualifications
Licensed as a RN in the State of Texas
Minimum of 1 year of experience in Hospice
Excellent written and verbal communication
Ability to think critically and work as part of a team
Ability to work efficiently in rapidly changing environments
Must demonstrate adequate skill in performing clinical procedures
