Client Care Coordinator/Scheduler Job Opening in Scottsdale, Arizona - Position Summary:
The Client Care Coordinator is the main point of contact for our clients and their family members. As the Client Care Coordinator, you will be responsible for supervising the care of our clients by assessing and monitoring the quality of services provided to them. This will be accomplished through on-site visits which are an essential role in the Client Care Coordinator position.
Duties include:
-Assess and monitor the services necessary to meet client needs
-Perform caregiver and client introductions upon first visit
-Conduct 30 day, 60 day, and quarterly supervisory visits of clients and caregivers
-Meet with clients regularly to re-evaluate care plans as needed
-Communicates with other Care Coordinator regarding client and caregiver schedules and needs
-Communicate with the HR Coordinator about staffing and employee needs
-Assists with new hire orientation training, quarterly training, and in-home visits
-Conducts caregiver 90 day and annual reviews
-Provide administrative support as needed
-Assembles client resource folders and goody bags
-Tracks Weekly Progress Report submissions and files into client folders
-Occasionally fill in on shifts when needed
-Responsible for overseeing all scheduling of your designated clients
-Stays up-to-date with all current caregiver and client issues
-Responsible for bringing employee issues and concerns to the HR Coordinator to determine appropriate action
-Creates client care plans through completed assessments. If the assessment is incomplete, it is the responsibility of the Client Care Coordinator to retrieve the necessary information from the clients.
-Reviews issues documented on Client Progress Reports
-Follows up on incident reports as needed within 24 hours of occurrence
-Roughly 75% office work and 25% field work
Job Requirements:
-High school diploma or GED
-Proven ability to supervise and mentor others
-5 years of related experience. Home health, homecare, assisted living, or healthcare experience such as an LPN, CNA or retired nurse preferred.
-Minimum 2 year experience providing care to seniors or persons with disabilities
-Proficiency with Microsoft Office, Outlook and web applications
-Previous experience with scheduling systems a plus
-Excellent organizational, customer service, telephone, and interpersonal skills
-Ability to connect and interact clearly with seniors
-Must have superior oral and written communication skills
-Ability to work under pressure and meet deadlines
-Position is Monday through Friday with a rotation of On-Call week nights and weekends
