Community Based Health Care Coordinator/ Hillsborough County Job Opening in Manchester, New hampshire - Our team is growing and we are looking for a Community Based Health Care Coordinator to work with children with special healthcare needs who reside in Hillsborough County.
A Care Coordinator assumes the overall responsibility for assisting the family in accessing appropriate services for the child. This is done through the development of a resource database, and knowledge of appropriate health and social services in the community. A competent case manager:
performs care coordinator functions within specified time frames;
maintains timely and accurate documentation of application, assessment, service plans, progress notes and other information required in each child?s record;
understands and subscribes to the philosophy and values of culturally-relevant education, empowerment and community integration;
works to ensure that the family makes informed decisions, and is in regular contact with the child and family to discuss progress, problems and plans;
communicates and collaborates with child?s health and related services providers, and documents in each child?s record.
Qualifications: RN, MSN, MSW, or allied health professional with solid pediatric/public health experience, knowledge of community resources, strong communication and advocacy skills.
