Job Description:
Applicants for this position, Outreach Specialist , must work in conjunction with all departments in order to deliver compassionate, high-quality, affordable health care information, in order to improve all communities quality of life. The Outreach Specialist will be directed and charged with engaging and informing vulnerable communities through various strategies, such as, but not limited to: door canvassing, phone banking, strategic mailing, special event tabling, and community presentations.
Abilities and Duties:
Ability work closely with neighborhood stakeholders and government entities.
Ability to provide CHA patients and the general public with a general description of CHA?s services and programs.
Ability to work with strategic partners on collaborative messaging and program coordination.
Ability to facilitate presentations regarding CHA?s programs, services, and pertinent healthcare information.
Must exhibit professionalism when directly engaging with the public.
Will be required to collect data survey information.
Will be expected to keep accurate records to maintain the integrity of research findings.
Knowledge of community resources
Skill in operating a computer/laptop utilizing word processing, presentations, databases, and e-mail
Skill in operating various office equipment, such as calculator, copy machine, fax, smart phone, and telephone system
Skill in accuracy and attention to detail
Ability to maintain confidentiality of financial and medical information according to Health Insurance Portability and Accountability Act (HIPAA) regulations
Ability to read, write legibly and calculate mathematical figures
Ability to solve practical problems and deal with a variety of issues
Ability to exercise good judgment in appraising situations and making decisions
Ability to work and interact effectively and positively with other staff members to build and to enhance teamwork in the clinics and overall Community Health Alliance (CHA) organization
Ability to communicate in a courteous and professional manner
Ability to understand and respond appropriately, effectively and sensitively to special population groups as defined by race, ethnicity, language, age, sex, etc.
Ability to hear and speak to converse over telephone and in-person
Ability to see to use computer efficiently and read computer reports and correspondence
Education and Experience: High School Diploma or GED equivalent required. Fluency in English and Spanish both verbal and written is a plus.

Provides translation services to other referral providers as needed as applicable especially pertaining to care coordination.

Participates in community functions that represent CHA.
Performs other duties as assigned.

Number of inbound and outbound calls handled daily and monthly.
Number of surveys processed daily and monthly.

Requires expeditious and accurate completion of tasks assigned to allow the successful completion of individual and corporate goals.
Follow-through and follow-up with time-sensitive information.
Maintain confidentiality of records in accordance with HIPPA and CHA policy. Keep all patient information in a locked file cabinet or drawer.
Communicate with immediate supervisor as to progress and/or issues impeding progress for successful completion of any assigned task.

Work with minimal supervision as a team member in a self-directed environment, adhering to the mission and values of CHA.
Attends required department and organization staff meetings in order to be an informed employee.
Meets deadlines related to projects, regulatory and organizational policies and practices and as directed by immediate supervisor.
Represents CHA in a positive, professional manner effectively communicating the mission and values of CHA to both internal staff and external clients.
Willingness to learn new skills and continuously improve processes as needed or required.
