HHA Role and Responsibilities:
Provide personal care for patients on multiple weekly visits
Document according to Plan of Care established by RN Case Manager
Communicate with RN Case Manager patient's clinical and psychosocial needs
Coordinate with HHA Manager and other HHAs to best meet the needs of all agency patients
