Clinical Liaison Nurse Job Opening in Lebanon, New hampshire - We currently have a fulltime (32 hr.) Clinical Liaison Nurse position to support Dartmouth-Hitchcock Hospital in Lebanon, NH.
The Clinical Liaison Nurse provides clinical assessment of patients referred by member hospitals for home infusion therapy to determine their capabilities to safely receive services at home with NELC. They are also responsible for relationship building with hospital staff for quality coordination of care between hospital and home. In addition, the Clinical Liaison Nurse is responsible for developing and implementing strategies to capture all referrals from the member hospital's system of care (e.g. ambulatory,inpatient, etc.)
This is a team oriented position in a fast-paced, challenging environment providing support to a multidisciplinary team of healthcare professionals.

Requirements:
Bachelors or Associates degree in Nursing
Minimum of 2 years acute care required
R.N. License in good standing in NH
Experience:
2 yrs. experience in Clinical Liaison role
Previous home health nursing preferred
Previous home infusion therapy preferred
