Clinical Coordinator - Every Woman Counts Job Opening in San Jose, California - Community Health Partnership was established in 1993 as a non-profit (501c) member organization representing community health center non-profit organizations throughout Santa Clara and San Mateo counties. CHP?s current membership is comprised of nine Community Health Centers (CHC) providing service at 38 sites, combined. Our mission is to advocate for affordable and accessible health services for our diverse and multicultural communities regardless of socioeconomic, ethnic, religious, or cultural background, and to support our member organizations in achieving these goals.
CHP is a regional contractor of the Every Woman Counts (EWC) program serving four counties in the Central Coast (Region 7): Santa Clara, San Benito, Santa Cruz, and Monterey. The program is funded through the California Department of Health Services Cancer Detection and Treatment Branch. The EWC program in Region 7 maintains a network of diverse primary care providers, and coordinates tailored breast and cervical cancer health education and outreach to eligible women in each county. The mission of EWC is to save lives by preventing and reducing the devastating effects of cancer for Californians through education, early detection, diagnosis and treatment.
POSITION OVERVIEW
Reporting to the Project Manager, the Clinical Coordinator is responsible for the coordination, implementation, and evaluation of Every Woman Counts (EWC) provider relations and patient services activities. The Clinical Coordinator promotes and administers the EWC program to primary care providers (PCPs), referred to as the Provider Network. The Clinical Coordinator manages the Provider Network, optimizing quality assurance and professional education and training to the Provider Network.
DUTIES AND RESPONSIBILITIES
Meet the clinical quality assurance service objectives of the EWC program.
Maintain and build a Provider Network within Region 7.
Assess Provider Network to maintain patient access to services.
Conduct training, site reviews and visits among Provider Network utilizing EWC standardized toolkits.
Provide technical assistance to Provider Network and patient navigation to EWC eligible/enrolled patients by utilizing in-depth knowledge of all clinical program components and manuals to ensure compliance with EWC policies and procedures.
Apply educational strategies for identifying the learning needs of clinic staff and conducting staff trainings that address these needs.
Maintain a resource referral toolkit with patient resources that address barriers to breast and cervical cancer screening and treatment.
Recruit providers to attend state-sponsored training.
Collect, maintain, and conduct program evaluation and data entry.
Prepare and submit reports to the Department of Health Services and others agencies.
Maintain communication with Provider Network regarding changes to program policy and procedures.
Support and advise individuals, public health agencies, clinic systems staff, and other local entities about EWC?s clinical and patient navigation services.
Other responsibilities as assigned to support Provider Network and to meet program mission, goals, and objectives.

Must possess a valid active license and be a Registered Nurse in good standing in the State of California (CA).
Minimum one-year experience working in clinical or community clinic setting.
Must possess a valid CA driver?s license and proof of insurance.
Ability to travel through the Central Coast region.
Mandatory knowledge, skills, and abilities:
Apply principles and practices of public health nursing and preventive medicine to EWC clinical activities.
Uphold the Health Insurance Portability and Accountability Act (HIPAA) of 1996. Understanding of applicable patient/provider consent laws and compliance with confidentiality policies
Ability to direct and instruct health care colleagues in a collaborative and constructive manner.
Excellent verbal, written, and communication skills.
Perform assessment, planning, implementation and evaluation of the clinical components of the EWC program Scope of Work (SOW).
Understand medical language, pathophysiology, and ethical/legal issues in healthcare, particularly in cancer screening, diagnosis, and treatment.
Current knowledge about health care and clinic management systems.
Understanding of principles of quality assurance to assure the PCPs adhere to EWC program standards and that EWC program benchmarks are met.
Knowledge of community resources in Santa Clara, San Benito, Santa Cruz, and Monterrey Counties.
Computer literacy (e.g., Microsoft Office Suite and database systems).
Competence to assume broad responsibilities requiring independence and professional judgment while conducting EWC program activities.
Experience working with diverse groups, particularly women.
Fluency in Spanish or Vietnamese highly desired.
