The IDEAL candidate will exhibit and have the following experience in:
MUST have a MA degree in a related field of study
Client Outreach and/or Residential experience
Mental Health Case Management and service and assistance experience working with the vulnerable adult population

Job Title : Roads to Community Living, Behavioral Health Specialist
Job Summary : Provides relocation support for individuals moving from institutional settings to community settings, while providing specific support for Challenging Behaviors and Transitional Mental Health and Substance Abuse for RCL participants.
Reports To : Full Life Care Solstice Behavioral Health Program Director / RCL Program Manager
FLSA Status : Non-exempt
Essential Job functions:
Conducting pre-transition consultation with participant to review RCL Service Authorization, and review participant needs and preferences;
Act as a liaison on behalf of HCS and AA staff, SNF staff, Housing providers, medical and legal providers, family members, informal support and other involved parties;
Providing assistance and support to ensure successful transition from institutional setting as authorized by AAA and/or HCS staff;
Provide direct interventions to decrease aggressive, destructive, sexually inappropriate, or other behaviors that compromise a participants ability to remain in the community;
Provide training, create behavioral support plans, and specialized cognitive counseling as needed to carry out successful transition;
Provide mental health and/or substance abuse services for participants whose needs are not otherwise covered by Medicaid;
Working with local housing authorities and other housing resources regarding housing availability, subsidies and voucher capacity;
Arranging of participant?s physical move from institutional setting;
Serving as a liaison between the participant and housing provider;
Accurately tracking and reporting purchases, filing receipts and other activities related to purchases made on behalf of RCL participants;
Assistance with developing personal skills related to the individual?s care plan including:
Helping the participant achieve measurable goals;
Helping the client access health services in a community setting;
Helping the client achieve personal skill development related to Service Plan tasks;
Self-Advocacy training;
Connecting participant with less formal or atypical daily activities such as those that might be found through local organizations like YMCAs or Senior Center;.
Communicating participant progress with HCS/AAA staff;
Completing reporting requirements such as time sheets, weekly/monthly reports and other documentation required for RCL participants;
Requirements Needed to Perform the Duties of the Job:
Ability to read, write and understand directions;
At least eighteen (18) years of age;
Must have access to private reliable means of transportation;
A minimum of two (2) years work experience in social services or related field;
Licensed and credentialed as a Mental Health Counselor, Marriage and Family Therapist, or Independent Clinical Social Worker or Associate with the state of Washington in accordance with the requirements of Chapter 18.225;
Ability to effectively use computer and communication technology;
Ability to work in various settings, including in a team setting with other RCL staff;
Ability to communicate with clients and other stakeholders in RCL project;
Demonstrates the following characteristics:

Able to work independently and communicate effectively with clients and supervisors;
Organized, self-motivated, honest and mature;
Able to perform data-entry tasks;
Ability to demonstrate skills or training specific to the care needs of patients or clients through skills observation by supervisor and/or evidence of previous training or experience;
Educational Requirements needed to perform the Duties of the job:
Master?s Degree in Social Work, Psychology, Education or related field;
