Case Manager / Social Worker Job Opening in Bellevue, Washington - First Choice In-Home Care is seeking a full-time Case Manager / Social Worker to manage a client caseload consisting of 50 to 100 individuals with a primary focus on consolidation of the service recipients in-home care team through the addition of qualified care providers. The ideal candidate is responsible, reliable, accountable, has passion for helping others that shows through in many ways and feels a strong motivation to undertake challenging work for the benefit of vulnerable children, adults and seniors.
First Choice is licensed by the Washington State DOH and contracted with DSHS to provide non-medical, in-home, personal care services to our population of developmentally disabled children & adults, individuals with mental health disorders and aging & elderly individuals. We provide services to both State funded (Medicaid) and Private clients living in King, Pierce and Snohomish counties. We are one of the most established In-Home Care Agencies in our service area and now manage the care of over 1,600 individuals.
We are growing rapidly and are now hiring qualified, passionate and motivated staff members into our King County Case Management Department.

Schedule: The position is full-time, 40 hours per/week, Monday-Friday 8:30am-5:00pm (30min break)
Qualifications, Education, and Experience:
Bachelor degree with emphasis: Psychology; Social Services, Sociology or related field is preferred.
RN's, LPN's, MA's and NAC's with 4+ years of work experience are encouraged to apply.
Ability to work effectively both independently and in a team environment.
Experience working or volunteering with disabled and/or vulnerable children, adults, and seniors.
Ability to problem solve and to pro-actively seek solutions.
Attention to detail and strong organizational skills.
Responsible driving record, valid automobile insurance and ability to pass a criminal background.
Previous experience is a plus!
Extensive training for all essential functions is provided.
Key Responsibilities & Competencies; include but are not limited to:
Managing a client caseload comprised of 50-100 minors/adults with disabilities and elderly/aging individuals.
Establish Client Care Teams, coordinate regular care regiment, assign and direct care providers.
Attend bi-weekly department meetings.
Attend weekly strategy meetings with intermediate team members.
Ensure Client health and safety is maintained through comprehensive supervision and oversight.
Coordinate with Human Resources Department, Training Department and Fiscal Department.
Conduct one-on-one Intake Assessments with new Clients.
Conduct semi-annual home visit meetings with existing Clients.
Complete administrative reports as needed and within stated deadline.
Utilize proactive and creative problem solving methods to address the items arising within assigned caseload.
Ability to prioritize team work and contribute to pressing needs outside of dedicated caseload assignment.
Detailed and extensive training is provided before receiving a Caseload Assignment
