Critical Care Pharmacist, Clinical Specialist Job Opening in Huntington, West virginia - Position Summary
The Critical Care Pharmacist, Clinical Specialist for our Pharmacy Department will:
Plans, organizes, coordinates and performs clinical activities in Critical Care Units
Facilitates and provides clinical pharmacy services, including coverage of clinical consults,
Code Blue responses, nutrition evaluations, patient education, etc., while balancing other hospital quality and cost ?containment initiatives.
Develops clinical services in Critical Care that includes assessment, monitoring and outcomes related to patient drug therapy per department policies and procedures.
Submits reports to Clinical Pharmacy Coordinator and director describing clinical interventions, results and trends, overall program progress and goals according to department policies.
Assists in the development of written policies and procedures of clinical pharmacy services in critical care to ensure the quality of the program.
Assists in the development and maintenance of the drug formulary to objectively facilitate the evaluation and selection of appropriate drug products.
Plans, develops and provides drug information services and in-service education programs for physicians, nurses and other health care professionals to enhance professional knowledge and patient care.
Precepts pharmacy students and PGY1 Pharmacy residents on clinical rotations.
Is an active member of the Pharmacy and Therapeutics Committee,Critical Care Committee, Trauma Committee, and other committees as deemed appropriate by the Pharmacy Clinical Coordinator to maintain working relationships with the physicians and other hospital staff.
Job/License Requirements
Qualified Pharmacist candidates will have a Doctor of Pharmacy degree
Pharmacist must have the ability to be licensed in WV
PGY1 Residency or 3 years of practice critical care settings.
Preference to Pharmacist applicants Critical Care PGY2 Specialty Residency and previous clinical experience.
