RN Case Manager Job Opening in Fairmont, West virginia - JOB SUMMARY:
Coordinate the care and services of patient populations across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management such as clinical practice guidelines and variance analysis.
Perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness of setting, clinical practice guideline monitoring, development of a plan, and patient and family teaching.
Timely and appropriate discharges will be accomplished through assessment of the patient?s post discharge needs and the provision of services necessary to meet those needs. Identify opportunities for improvement in patient care and facilitate patient care support functions provided to neonatal, pediatric, adolescent, adult and geriatric patients.
Demonstrates excellent customer service to both internal and external customers through positive communication, cooperation, professional behavior, and displaying by actions that customer satisfaction is the primary goal of this institution.
Adapts to constantly changing healthcare environment by recognizing opportunities for improvements. Exercises independent thinking, judgement, and decision-making and accepts accountability and responsibility for decisions and consequences.
Complies with all policy & procedures and related protocols as established by the
Department and/or the organization.
Collaborates with existing programs and departments to ensure appropriate resource
Utilization by all caseload patient.
Attends, participates and leads interdisciplinary team meetings as appropriate.
Facilitates discharge planning for acute care patients.
Conducts formal and informal staff education related to case management, outcome analysis, , and reimbursement.
Facilitates clinical practice guideline compliance utilizing physician and staff input.
Identifies and documents variances as a member of the healthcare team using case reviews.
Performs utilization management, discharge planning and resource management in accordance with state and federal guidelines and policy and procedures.
Demonstrates excellent skills in all forms of communication.
Reviews patient observation status to assure appropriate and timely disposition of patient.
Knowledge of QIO guidelines and Medicare rules and regulations.
EDUCATION, EXPERIENCE, TRAINING
Current Registered Nurse License from State of West Virginia required, graduate Bachelor of Science in Nursing preferred, hospital case management with Inter Qual skills set required.
Three years acute care nursing experience, critical care/ER experience preferred, previous case management experience preferred, certification in case management preferred.
CPR certification
Highly motivated, assertive, computer skills necessary, knowledgeable in community resources and alternate care facilities, demonstrates exceptional written and verbal communication skills.
