Social Worker Job Opening in Pell City, Alabama - Purpose:
To assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional, and spiritual programs, in accordance with the resident?s assessment and care plan.
Qualifications:
Bachelors Degree from an approved school of Social Work for a certain size facility.
Must have one year supervised social work experience in a health care setting working directly with individuals.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the resident served.
Must possess the ability to make independent decisions when circumstances warrant.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must have patience, tact, cheerful disposition and enthusiasm.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
Must be able to relate information concerning a resident?s condition.
Essential Functions:
Assist in planning, developing, organizing, implementing, and evaluating the social services programs of this facility.
Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
Participate in discharge planning, development and implementation of social care plans and resident assessments.
Interview resident or family members to obtain social history.
Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
Evaluate social and family information and assist in determining plans for social treatment.
Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
Participate in facility surveys (inspections) made by authorized government agencies as necessary.
Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the service.
Serve on, participate in, and attend various committees of the facility as required.
Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
