Highland Healthcare is seeking a Registered Nurses, Licensed Practical Nurses, or Licensed Social workers interested in making a real difference in the lives of those they serve. In partnership with a nationwide insurance carrier, we provide home visits to those at risk for injury, illness, or hospitalization to provide needed tools to promote comfort and well-being at home. This is a deeply gratifying, self-directed role in the rapidly growing environment of population-targeted healthcare.
MINIMUM REQUIREMENTS
Registered Nurse with minimum 1 year experience relevant case management or home care.
Licensed Practical Nurse with minimum 3 years relevant case management or home care.
Licensed Social Worker with minimum 2 years relevant case management or home care.
Self-directed, motivated, autonomous.
Excellent communication skills
Intermediate computer skills
Experience with complex case management of chronically ill, functionally challenged and geriatric populations.
Ability to complete self study and meet timelines.
COMPENSATION is based on visits and documentation
