Care Services Coordinator Job Opening in Salt Lake City, Utah - Summary:
Provide care and support to those living with ALS and their families by facilitating education, information, and resource navigation assistance to people living with ALS and their caregivers. Develop professional and community relationships to enhance the services provided by the chapter.
Classification: 40 hour/week professional. Working from remote home office, including daily and potential overnight travel and occasional weekend work. Health insurance provided. Service area is Utah.
Duties:
Respond to patient and family inquiries for information and referrals, including all referrals received from the Chapter Office, for service region
Develop and maintain information on local resources
Maintain updated information on ALS, ALS specific research, patient care techniques, and equipment
Schedule home visits or in-person meetings as appropriate
Maintain accurate records on patient visits and interactions
Assist with the management of local equipment loan program
Facilitate, maintain and develop support groups
Attend chapter events and committee meetings as appropriate
Assist with the development of chapter events as needed
Other responsibilities as assigned or identified through ongoing analyses and community needs
Essential Qualifications:
Bachelor?s Degree in health related field; preferably RN, SW or allied health professional
Therapy experience preferable - PT, OT, Social Work, CODA, etc.
Three years of related professional experience
Strong analytical and problem solving skills
Superior written and oral communication skills, including public in-service presentation experience
Ability to successfully identify and cultivate relationships with a wide range of stakeholders
Self-management skills; ability to achieve organizational goals while working in remote environment
Ability to contribute value to team projects; individually and as a group member
Ability to successfully participate in distance learning
Display strong work ethic, personal integrity, empathy
Valid Utah Driver License with clean driving record
Ability to drive locally, travel within Utah, work occasional evenings and/or weekends
Ability to pass a drug screen and a criminal background check
