The Registered Nurse (RN) Case Manager works directly with the RN Clinical Supervisor, under Physician orders for the overall management of our clients to including the provision of direct care with a designated visit productivity standard. The RN Case Manager plans, organizes, and directs all client care services for clients with in their assigned caseload in accordance with current standards and regulations of the state and agency. This role manages and coordinates the clinical staff of each plan of care to implement the established plan ensuring effective management of visit utilization client care and control of expenditures.
Professional Responsibilities of the RN Case Manager
Visit clients in their homes 1:1 nursing with In-home Clients, skilled nursing and non-skilled (ADLs supervision) and provide cares based on physician orders and following care plans
Responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting
Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
Support junior staff members with continued education, ADLs and medication reviews; offers networks to resources per agency, state and federal guidelines
Support clients and families with the transition from a care facility back to their home as needed to insure the maintenance of or acquisition of optimal functioning level for each client
Guarantees effective coordination of client home care services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the client's physician, therapists or other care givers
Routinely reviews pertinent client medical data to determine the effectiveness of services in reaching maximum rehab potential
Performs client intake assessments including submission of required documentation for prior authorization of client services as needed based on client's pay sources
Assures agency intake processes meet applicable local, state or federal requirements in addition to agency policies and procedures
Specific Requirements of the RN Case Manager
Current Registered Nurse (RN) license in good standing in the state of application
Strong assessment and organizational skills
Ability to Multi-task and work under time constraints
Oasis or ICD-10 a plus but not required
