We are currently accepting qualified applications for an Inpatient Pharmacist and Clinical Pharmacist to support one of our Government clients at beautiful Kessler AFB, MS.
Experience:
Inpatient Pharmacist:
Must be licensed for a minimum of two years, with a minimum of 12 months of consecutive, full-time inpatient pharmacist experience within the last 36 months in an inpatient hospital setting, or
Have graduated from a college of pharmacy or residency program accredited by the American Council on Pharmaceutical Education (ACPE) within the last 12 months.
Clinical Pharmacist:
Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties, or Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy.
Specific procedures for which the Pharmacist shall be responsible:
The duties include but are not limited to the following:
Perform a full range of pharmacist responsibilities.
Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
Counsel patients regarding appropriate use of medications.
Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items.
Compound medications as required.
Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
Work closely with clinical staff in developing patient wellness programs and providing medication management therapy.
Provide medication information and medication therapy guidelines to the clinical staff as required.
Provide medication and healthcare related in-service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
Provide clinical direction to pharmacy personnel.
Supervise inventory control of all pharmaceuticals in the pharmacy, to include scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
Consult with other specialty practitioners to provide optimal pharmaceutical care.
Ensure that work areas are clean and safe according to applicable regulations.
Document significant medication interactions and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with TJC standards as appropriate.
Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
Provide clinical direction and evaluate care provided by other outpatient pharmacy employees and students assigned.
Participate in various committees; regular and prompt attendance to Flight, Squadron and Group meetings as required.
Stay abreast of policies of regulating authorities including TJC, Air Force Health Services Inspections and federal and state regulations.
Safely perform clinical pharmaceutical care functions and adheres to current infection Control and Safety Standards.
Perform quality improvement duties including performing drug storage inspection, reviewing expired supplies, and producing error and workload reports and documentation.
Perform supply process duties including placing new orders, stocking/restocking shelves, inventory maintenance and out of stock reports.
A working knowledge of pharmacy computer systems such as CHCS, Essentris, PharmASSIST and Pyxis computer systems.
Be in the work area and available for work at the appointed times.
Be productive and perform with minimal supervisory direction.
Complete all job specific and Air Force training as required by the MTF.
Perform any other assigned duties as required.
The duties include but are not limited to the following for the Clinical Pharmacist:
Assess patient?s response to drug therapy and planning drug therapy based on diagnoses.
Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., medication therapy management, hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotics).
Initiate or modify drug therapy for minor acute conditions such as colds, rashes, and allergies.
Administer prescription or non-prescription drugs according to established agreements or protocols.
Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
Conduct and coordinate clinical investigations and research (consistent with other healthcare professionals) approved by a local or regional Investigational Review Board (IRB) and participating in outcome studies generated by the Pharmacy Flight and approved by the P&T Committee.

Formal Education:
Inpatient and Clinical Pharmacists shall have:
