Patient Care Coordinator - Stuart, FL Job Opening in West Palm Beach, Florida - Medical Billing Group is a rapidly growing healthcare revenue optimization company. Our offerings include claims denials recovery and management, Chronic Care Management, Annual Wellness Visit and full Revenue Cycle Management solutions for physicians and hospitals. We utilize leading edge technology to support our team members who provide service to our clients.
We are looking for a Care Coordinator (CNA, CMA, LPN) to deliver Chronic Care Management (CCM) and Annual Wellness Visit services to Medicare patients in a physician office. The Care Coordinator will be responsible for:
Identifying Medicare eligible patients
Conducting Annual Wellness Visits
Taking vitals
Enrolling patients in Chronic Care Management program
Creating Care Plans as well as Advanced Care Plans (ACP) to enable payments from Medicare
Documenting care coordination activities for CCM
Reviewing Care Plans and Personalized Preventative Plans with patients
Educating patients on their personalized health plan
Patient outreach
Scheduling appointments
Attaching supporting documentation in patient charts (EHR) to enable payments from Medicare
Communicating with providers regarding patient activities in the program
Required Skills and Qualifications:
Certified Medical Assistant, Licensed Vocational Nurse, or Licensed Practical Nurse
Excellent communication (phone) skills
Proficiency working in EHR systems
Ability to effectively interact, in both oral and written forms, with staff, physicians, patients, and families
Operational knowledge of MS Office (Word, Excel, PowerPoint) and Adobe Acrobat
Detail-oriented with excellent organizational skills
Ability to meet productivity standards with minimal supervision
Salary range based on experience $18-23/hour
