Care Coordinator Job Opening in Clearwater, Florida - This is a customer-service and leadership-focused position working directly with patients and their families, insurance
representatives, doctors and other medical personnel in a dynamic and professional environment to provide the highest
level of quality healthcare to all patients.
1. Provide extraordinary customer service to all internal and external customers (including patients and other
Chenmed Medical team members) at all times.
2. Ensuring the completeness and accuracy of all tasks.
3. Address / resolve any customer-service issues.
4. Ensure Accuracy/ Existence of Insurance Eligibility.
5. Ensure proper flow of patients from check-out area.
6. Follow up on all Home health and DME orders to ensure patient receives services ordered.
7. Retrieval of consults notes via consult tracking tool.
8. Ensure patients missed appointment are reschedule and communicate to physician missed appointments.
9. Checkout patients based on their assigned PCP. (Note: If care coordinator not available the next care coordinator
may assist patient. Additionally this process does not apply to specialist)
10. Address all Referral based phone calls for Primary Care Physicians panel.
11. Prioritizing HPP patients in Primary Care Physicians panel.
12. Utilize incoming referrals process for external order request.
13. Prepare and be an active participant during PCP huddles utilizing RITS.
14. Completion of orders with proper documentation on where patient is scheduled and how patient was informed.
15. Orders older than five days must be documented with details.
16. Use of webivr to generate authorizations ( Availity, Careplus, Healthhelp NIA and any other implemented web ivr
for authorization processing.)
17. Entering all Inpatient and Outpatient procedures in HITS tool.
18. Communicate effectively to Primary Care Physician needs or outstanding items in regards to patients.
19. Proper Completion of Orders.
Definition of completion: - Notification is sent (if needed) - Order has been approved (if needed) - Patient has been informed of appointment - Referral has been sent to specialist office & confirmed receipt
Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
· Knowledge of JenCare /Chenmed Medical products, services, standards, policies and procedures · Skill in operating phones, personal computer, software and other IT systems · Skill in oral communication (English required) · Ability to communicate with team members, patients and other individuals in a professional and courteous manner · Ability to pay close attention to detail and to ensure accuracy of reports and data · Ability to perform multiple processes in the Referrals Department
EDUCATION / SPECIALIZED KNOWLEDGE REQUIREMENTS:
· High School Diploma or equivalent
