As a Community Liaison, you will:
Educate those in the healthcare field about our home care services
Develop and maintain relationships with healthcare professionals
Communicate needs and benefits of home care services in the community
Coordinate new referral admission process to ease transition
Follow-up with referrals to assess services

Qualifications:
Bachelor?s Degree in a business related field
